Using this website

This website provides information about Lewis Health Insurance and ways to contact the agency. It does not offer online enrollment, and it does not ask for medical or insurance identification information.

Contact form information

When you submit the contact form, we use your name, contact details, preferred contact method, topic, message, and consent to respond to your request. Submissions are sent to Shannon Lewis at shannon@lewishealthwa.com using Amazon Simple Email Service. Please do not submit medical information, Social Security numbers, or Medicare numbers.

Protecting the contact form

Cloudflare Turnstile checks submissions to help prevent spam and abuse. Cloudflare processes technical information, such as your IP address and browser information, for this check. Form messages are processed by our hosting and email providers and delivered to the agency’s inbox. Contact the agency with questions about your information or to request a correction or deletion.

Messages in agency email

Contact form messages are delivered to the agency’s email inbox. This website does not maintain a separate database of inquiries. Ask Shannon about how long the agency keeps messages or to request a correction or deletion.

Email and telephone contact

If you contact the agency by email or telephone, you share information directly through that service. Please do not include Social Security numbers, Medicare numbers, or medical details in a general email. Ask Shannon how to share information needed for your appointment.
